Instructions:
1. Complete all forms from the links above and make a check for the appropriate amount (depending on the date post marked); payable to Premier VBC. Each team only needs to submit one team registration form, however, each player must submit both an individual registration form and the Arena Sports Waiver form. If an Arena Sports Waiver has already been submitted for the camp, then an additional waiver does not need to be submitted for the league.
2. Mail the completed forms and the check to:

3. When your check and completed league forms are received you are registered.
